The outreach caseworker will provide social and enabling services to all NVHC homeless and at-risk of homelessness population. Outreach caseworker will tackle both minor/major social challenges by
working with and alongside local community HCH/non-HCH programs to provide the best resource/service to NVHC patients. Working at the core of the healthcare for the homeless program to understand issues relating to homelessness and at-risk populations, to identify, engage and support NVHC/community-led actions to address social determinants of health that primarily affect the city’s most vulnerable population. Provider support, resources, and referrals to those who require shelter, finding employment, and obtaining medical services. Act as the liaison to promote access to NVHC mental health services.
General Duties
Coordinate social service needs, in collaboration with the NVHC Behavioral health team.
Transport patients to and from specialty visits/external services as referred by NVHC providers and/or social workers.
Transport patients to and from their NVHC medical visits.
Network and provide linkages with new and existing agencies.
Document and compile statistical and summarized monthly/quarterly reports.
Advocate on patient's behalf for all social/medical needs.
Attend community meetings to represent the HCH program and various task forces,coalitions’ relevant issues affecting homelessness.
Maintain a high level of professional conduct, which adheres to NVHC policies andstandards for behavior.
Develop professional and supportive relationships with the homeless or at-risk personsstaying in outdoor locations or other places not meant for human habitation in the Las Vegas area.
Utilize an outreach schedule to contact, interview, screen, and refer clients to appropriate agencies for assistance
Provide information regarding services and transportation as needed to those seeking services
Make/conduct presentations to interested agencies concerning the outreach program
Identify and document clients’ needs through repeated contacts with homeless persons
Efficient in Homeless Management Information System (HMIS) and provide daily documentation.
Ability to maintain confidentiality and follow all HIPAA guidelines.
Must maintain a driver's license, drive safely and perform transportation.
Knowledge of outreach activities and issues surrounding homeless/at-risk population groups
Knowledge of essential functions of a medical clinic
Knowledge of general office practices and procedures
Skill in the operating computer utilizing spreadsheets, databases, and email, preferred experience with HMIS system.
Skill in accuracy and attention to detail
Must be insurable and provide own transportation to and from work. Must be able to drive LVHAWC Van
Must have a clean driving record and valid NV driver’s license required;
Ability to use good judgment in appraising situations and making decisions
Ability to understand and respond appropriately, effectively, and sensitively to particular population groups as defined by race, ethnicity, language, age, sex, or sexual orientation.
